Clinical experience in a case of exogenous lipoid pneumonia is reported. In this clinical case, the accumulation of exogenous lipid (paraffin oil) in the bronchial trunk in a tracheostomized female patient is underlined. The patient has been submitted to X-ray and TC of the thorax with transthoracic needle biopsy, bronchoalveolar lavage, gas-analysis and spirometry. After 3 and 6 months from the suspension of medicament and of antibiotic and anti-inflammatory therapy, the patient has been controlled with X-ray, TC and bronchoalveolar lavage that showed an improvement of the clinical, radiological and functional picture.
