The separation of a bipartite patella combined with quadriceps tendon rupture is a rare injury. Up to now, only 2 cases are reported in the literature. In this report we describe the case of a man, 71 years old and with no underlying disease, who presented diagnostic difficulties. The treatment of quadriceps tendon suture to the patella and excision of the displaced patellar fragment achieved a good recovery without any effect on knee extension.
