Osteoid osteoma is a benign tumour with predominant osteoblastic phenotype characterized by limited dimensions (<1 cm) which occurs with always deeper and continuous pain, usually stronger during the night. Osteoid osteoma is a common lesion and represents about 12% of the bone benign tumours. The localization of the osteoid osteoma is widely distributed, but unique. This tumour is characterized by the increase of the soft tissues. Diagnosis is based on X-ray, MR, TC and three-phase total body scan, and other clinical instruments. At present, the main therapy consists of percutaneous radiofrequency ablation. The clinical case we present fully describes an osteoid osteoma of the radium distal epiphysis, its general picture and diagnosis and it underlines that some time a traditional open surgery is needed due to failure of the percutaneous radiofrequency ablation. On the basis of this clinical case it appears that, although the good results described in literature on thermocoagulation, there are some localizations in which the traditional open surgery is still effective since it makes easier the complete removal of the neoplasm.
