Main polyethylene-related topics are discussed: a) the effects of high energy irradiation during the sterilisation, the consequent changes of the chemico-physical properties and the reduction of the molecular weight; b) the interaction with oxygen during the manipulation, the storage and the in vivo service of the implants and the oxidation; c) the cellular reaction to the wear debris and the aseptic loosening; d) the crosslinking process and its effects on the wear resistance and on the other mechanical properties; e) the roughness of the articulating surface and the choice of metal or ceramic inserts; f) the problem of the packaging and the storage of the inserts; g) future expectations, like the use of vitamin E for stabilisation and antioxidation of the polyethylene.
