Treatment of late results of distal humeral fractures with transosseous osteosynthesis

Fractures and epiphyseal separations of the distal humerus in children can lead to a malalignement deformity of the elbow (varus, valgus or complex deformity), which may require further surgical procedures due to aesthetic or functional impairment.
Among the surgical options, the circular external fixation (transosseous osteosynthesis) offers several advantages: there is a minimal dissection of soft tissues and a minimal devascularization of bone fragments, a progressive correction of the deformity can be performed, the ROM of the elbow is preserved and the frame is easily removed at the end of the treatment.
At the Orthopaedic Department of the University of Pavia, 8 cases of distal humerus deformity have been treated by supracondylar osteotomy, circular external fixation and progressive correction.
The good results obtained let us consider this method as a good alternative to more conventional techniques.
