Majority of orbital blow outs resulted from zygomatic-maxillary complex fractures and incidence of isolated orbital blow out fractures are rare. Herniation of orbital content to maxillary sinus is ever challenging surgically as retraction of orbital content should be done with mild and controlled force, avoiding oculi-cardiac reflex and damage to orbital content. This article highlights the use of sterile 5-cc spoon (teaspoon) with a modification as an ophthalmological retractor successfully, in orbital roof and floor reconstruction which is a frequent practice in Dental Hospital Peradneiya, Sri Lanka.
