Chest pain in pregnancy can be alarmingly difficult to manage. Life threatening causes such as ischemic heart disease (IHD) must be excluded especially in those with high-risk group although the incidence in pregnancy is low. Early recognition of this condition with prompt revascularization treatment is the key to prevent devastating sequelae. However, the diagnostic dilemma in the disease confirmation during pregnancy may lead to unnecessary further intervention. We report a case of chest pain during pregnancy with significant non-invasive cardiac testing who was treated for gastro-esophageal reflux disease later following negative angiogram result.
