This review will explore: 1) A summary of endothelial physiology and pathophysiology, 2) the identification of patients at increased risk for endothelial dysfunction, 3) the impact of endothelial dysfunction on perioperative morbidity and mortality, and 4) potential strategies aimed at optimizing endothelial function in the perioperative period, thereby targeting improved postoperative outcome.
