A case of acute appendicitis occurring in the right strangulated femoral hernia sac in a 83-year-old woman is described for its rarity: 59 cases were found in the literature until 1974. All the cases operated on for strangulated femoral hernia at the Milan Niguarda Ca’ Granda Hospital during a period of 20 years (January 1979-December 1998) have been reviewed. The diagnosis was established only peroperatively due to extremely uncommon condition, in an old woman. The literature findings on rarity, diagnostic difficulty and treatment easiness are confirmed.
