Perianal extramammary Paget’s disease (EMPD) is a rare disorder of unknown etiology and is frequently associated with malignancy. It is an uncommon intraepithelial adenocarcinoma primary affecting apocrine gland-bearing skin. The disease starts insidiously; lesions develop to well demarcated plaques, variable in diameter, red or brown coloured, with variable surface, usually peppered. EMPD may coexist with underlying adenocarcinoma, so a correct procedure needs search for tumours. The authors relate a case of perianal EMPD for its rarity and complex and multidisciplinary management.
