Perforation of a duodenal diverticulum is a rare and serious complication. The clinical case deals with the case of a 74-year-old woman with onset of acute abdomen. Diagnosis of duodenal perforation is performed, treated with emergency surgery. With exploratory laparotomy, a para-vaterian duodenal diverticulum was revealed and its exeresis was performed. The postoperative course was characterized by the stenosis of the papilla of Vater, treated in a multidisciplinary way, with placement of biliary prosthesis in order to create a neo-papilla.
