Iatrogenic bile duct injuries are still a diagnostic and therapeutic problem and their incidence increased with the introduction of laparoscopic cholecystectomy. This case report documents a patient who developed high-level bile duct injury due to laparoscopic cholecystectomy performed in another center and underwent an extraordinary repair that did not have a chance of positive outcome. This irrational reconstruction with panic and desperation made the patient’s condition more complicated, but thanks to the cooperation of a general surgeon and an interventional radiologist, successful management was performed and a good outcome was achieved in the long term. For this reason, in case of high-level bile duct injury, surgeons should avoid spontaneous decisions and attempts, focus on the most appropriate option for the patient, and should transfer the patient to a reference center without delay and any other intervention.
