Off-pump coronary artery bypass grafting, an unusual cause of systolic anterior motion of the mitral valve

We present a case of systolic anterior motion of the mitral valve (SAM) in a 73-year-old woman who underwent an uneventful off-pump coronary artery bypass grafting (OPCAB). SAM, left ventricular outlet obstruction (LVOTO), and mitral regurgitaion (MR) were deteriorated by Valsalva maneuver and improved by carvedilol in dose-dependent manner.
