We report a very rare case report of a 83-year-old woman with symptoms of cough, chest pain and shortness of breath who had a capsulated string-of-pearl-type bilateral giant posterior mediastinal masses (connecting to each other in front of the vertebra: right,7x5 cm and left, 3x4 cm) with massive symptomatic pleural effusion. It was straightforwardly resected using “an uniportal VATS approach” with an uneventful postoperative period.
