Congenital partial pericardial defect with left ventricle herniation is a rare anomaly. Patients may be asymptomatic, but chest pain, emboli, arrhythmia, and sudden death have been described. We report a 30-year-old male presented with palpitation and intermittent vague chest pain localized in the left precordium. Magnetic resonance imaging identified the PPD. The patient underwent patch repair of the pericardial defect with polytetrafluoroethylene. He had an uneventful recovery and the symptoms subsided after the operation.
