We report a rare case of a true primary mediastinal goiter associated with a papillary thyroid cancer in the cervical gland, located in a difficult para-vascular position (aorto-pulmonary window), and successfully resected through a three ports left thoracoscopy, using the intra-operative ultrasound guidance and the laparoscopic vessel sealing generator.
