The new tool to overcome fibrous binding lead sites, the EvolutionTM Mechanical Dilator Sheath (Cook Medical) is very useful for implantable cardioverter defibrillator (ICD) and pacemaker right ventricular lead extraction. We report a major cardiac complication after the use of this new tool for transvenous lead extraction.
