The intravenous leiomyomatosis (IVL) is a rare form of benign tumour that originates from smooth muscle cells and normally involves the pelvic venous system. Rarelly intracaval and intracardiac extension have been described. We report the case of 73 years old woman who underwent cardiosurgery in 1989 removing a leiomyomatosis mass from right atrium and in 2007 underwent a total hysterectomy with bilateral annessectomy removing the remaing tumoral mass.
