Aortic dissection, intramural hematoma and penetrating atherosclerotic ulcer are distinct groups of aortic pathology. We present a case of concomitant intramural hematoma and penetrating atherosclerotic ulcer in the ascending thoracic aorta, and review the literature on the current diagnostic and therapeutic recommendation.
