Tarsal synostosis in the adult: a not easy or prompt diagnosis

An interesting case of a 36-year-old woman with a calcaneo-navicular coalition in a peroneal spastic flat foot, wrongly diagnosed as calcaneal fracture sequel made by another orthopedic surgeon, is reported. Using the Downey's surgical classification, this lesion was treated with the fusion of the subtalar and Chopart's joints. The aim of this paper is to recommend to consider tarsal synostosis in adult patients suffering from tarsal pain, unresponsive to conservative treatment, particularly if associated with contracted valgus-flat foot.
