Traumatic dislocation of the first metatarsophalangeal joint are rarely reported cases, and injuries of the sesamoidal complex are much less common. A case of a twenty-day-old traumatic dislocation of the first metatarsophalangeal joint combined with a divergent luxation of the sesamoids and interposition of the flexor hallucis longus tendon is reported. The mechanism and classification of injury, the anatomy and their surgical procedure are described. A short review of the world literature is made and a rational for open reduction in each case of divergent luxation of the sesamoid bones is proposed.
