Lipomas are benign tumors frequently detected with a head and neck occurrence estimated in about 20%. Posterior subcutaneous neck localization is the most frequent, while deep parotid lobe localization is extremely rare. Surgical resection is difficult and requires complete facial nerve identification and dissection. Although total or near total parotidectomies are often suggested in the management of these tumors, selective deep lobe parotidectomy with complete preservation of superficial lobe could be considered an effective and safe approach that ensures safe facial nerve dissection, low functional impairments, and good cosmetic outcomes. In this paper authors present the case of a 47 years old male patient with a giant lipoma of the deep parotid lobe, discussing surgical management and results.
