Chondromyxoid fibroma (CMF) is a rare benign tumor of bone, mainly occurring in the metaphysial region of long bones. Localization to head and neck region is much rarer. We present here the second case of CMF of the zygoma described in the literature, arising in a 79-year-old woman. The patient was treated with block resection of the lesion and immediate reconstruction of the zygomatic area. The patient has remained disease free with regular clinical and radiographic follow-up for 40 months. Based upon this case and the other reported, CMF best treatment seems to be block resection for the significant recurrence rate following curettage.
