This article discusses the use of a transoral approach by Le Fort I osteotomy with palatal splitting to access the naso-ethmoidal complex. We report 2 cases of expansive lesions of the mid-face (juvenile ossifying fibroma and cervicofacial actinomycosis) in which this approach has been used. This approach is more suitable than the transethmoidal approach in the treatment of expansive lesions involving the mid-face. It is a relatively simple technique with low morbility and morbidity for the patient and it provides a good operative working area. The authors report the surgical technique, a brief history, directions how to use Le Fort I osteotomy and its effectiveness in the treatment of expansive lesions of the mid-face.
