Effect of hyaluronic acid on mucocillary clearance after septoplasty

BACKGROUND: Mucociliary clearance represents a host defense mechanism, that can be compromised by various pathologic conditions, as septal deviation, although septoplasty can resolve nasal obstruction, but uncertainty about its effect on mucociliary clearance, this study aims to demonstrate the impact of hyaluronic acid on mucociliary clearance, following septoplasy.
METHODS: Twenty-four eligible septoplasty patients were recruited in this randomized control trail, were the assessment of mucociliary function done through, saccharine time test preoperatively, and postoperatively (10 days, 4 weeks, and 6 weeks). The patients were divided evenly into; group “A” used nasal saline wash (control group), and group “B” used hyaluronic acid, both groups were followed up, after septoplasty, and then various clinical parameters were assessed and compared.
RESULTS: Mean SD of saccharine time test preoperatively was (Rt. 11.1±7.1, Lt. 14.4±7.5) in group “A,” and (Rt. 14.8±3.3, Lt. 13.2±5.0) in group “B,” then increased on 10 days postoperatively to (Rt. 21.4±8.5, Lt. 23.7±10.2) in group “A,” and (Rt. 23.8±4.3, Lt. 23.3±3.5) in group “B,” but it remarkably reduced after 6 weeks postoperatively, being in group “B” (Rt. 7.5±5.1, Lt. 7.5±3.2), compared, to group “A” that, took a longer time “slower healing process” (Rt. 14.3±5.5, Lt 17.3±5.5) with P=0.0001.
CONCLUSIONS: Hyaluronic acid spray had a positive effect on mucociliary clearance function, after septoplasty, as it accelerates nasal mucosa healing, with best improvement of postnasal symptom, also prolongation of mucocilliary clearance time in concave side compared to convex side of nasal septum.
