We report a case of a 50-year-old man with a vascular leiomyoma originating from the nasal dorsum. Vascular leiomyoma of the nasal cavity and paranasal sinuses are rare and usually appears as a small painless mass. Up to our knowledge, this is the first case in which a MRI scan was performed preoperatively showing very accurately the delineation of the lesion and helping to establish the surgical excision as the treatment of choice. This case of vascular leiomyoma located in the nasal dorsum has been successfully treated through an open nasal approach which provided to a safety exposure for the completely excision of the lesion with satisfactory functional and aesthetic results.
