This study reports a case of left maxillary and ethmoid sinus cavernous hemangioma with rare presentation involving bone erosion. Authors discuss the diagnostic and treatment modality, with emphasis on the clinical appearance, radiographic features and histological characteristics of the lesion. Hemangiomas of the paranasal sinuses are rare. Imaging is fundamental to determine the extent of involvement but final diagnosis is based on the histological appearance of the lesion. Obtaining an adequate biopsy can be difficult due to the risk of bleeding. In the present case substantial bone destruction and resultant widening of the left osteomeatal complex made it difficult to differentiate this cavernous hemangioma from malignant epithelial tumors. These obstacles can make the diagnosis and management of these lesions particularly challenging.
