Four main localizations of ectopic thyroid in the upper airway-digestive tract have been described: lingual, sublingual, prelaryngeal and substernal. The case of a 62 year-old man with an ectopic thyroid, who presented with symptoms of dysphonia and dyspnea is described. Examination of the neck showed a palpable mass in the anterior left wall that was not mobile on swallowing. Ultrasound examination, computed tomography and magnetic resonance showed a well defined mass causing the thyroid cartilage interruption. The embryology, diagnosis and management of this tumor are discussed.
