A case of solitary extramedullary plasmacytoma of the maxillary sinus

Solitary extramedullary plasmacytoma is a rare tumor due to neoplastic proliferation of plasma cells. It may be present as a strewn form (multiple myeloma) and localized form (solitary bone plasmacytoma and medullary plasmacytoma). The medullary plasmacytoma may be found in nose and paranasal sinuses, tonsils, oropharynx, larynx and more rarely in salivary glands, thyroid gland, temporal bone and lymphnodes of the neck. This tumor may be treated by radiotherapy and by surgery followed by radiotherapy. The patient was complaing from 10 days pain in the left maxillary region and obstruction of the left nostril due to edema of mucosa and to neoplastic tissue present in middle meatus; histologic exam revealed plasmacytoma. Bence-Jones’s protein was present. A neoformation located in left maxillary sinus with extention toward the premaxillary and maxillary region, orbit, ethmoid and hard palate that was completely missing. The patient was submitted to removal of neoplasm by paralateronasal approach and radiotherapy (6000 Gy). This tumor is surely rare; the symptomatology was the same reported by other authors; opposite, the presence of Bence-Jones’s Protein in urin is rare (10-20%). In the case presented, surgery was performed followed by radiotherapy due to the large extention of neoplasm.
