Nevus sebaceus of Jadassohn is a particular organoid nevus, arising from the pluripotential germinative cells in the basal layer of the embryonic epidermis. It’s almost always solitary and usually situated on the scalp. The lesion may develop benign and malignant neoplasms, usually with advancing age. Prophylactic excision has been advocated, before puberty, to prevent the occurrence of neoplastic tranformation. A case of a 20 year-old male from Morocco with a pigmented basal-cell carcinoma, arised in a large nevus sebaceus of the neck, is reported. Carcinoma was completely contained in the incisional biopsy and the nevus sebaceus was ablated with surgical excision in two steps. The uncommon localization of nevus sebaceus, its unusual size and the early development of neoplasm are underlined.
