Aortic arch pathologies have been controversially discussed within the scientific literature during the last decade. Apart from open surgical repair, interventional solution including hybrid treatment have revolutionized this subject. The purpose of this review was to give an overview on current treatment options of aortic arch pathologies. A PubMed search for the period from 2009 to 2014 was conducted with the following key words: “aortic arch repair”, “aortic arch replacement”, and “rerouting”, “debranching”, “hybrid”, “type a dissections”, “elephant trunk”, “TEVAR”. Hybrid procedures represent the most evolving development in treatment of complex aortic arch pathologies and type a dissections. Combination of surgical approach and endovascular intervention will guide the future perspectives in aortic surgery. Although the results of hybrid procedures are promising, long-term data in durability and outcome are missing, therefore the hybrid procedure is reserved for a carefully evaluated patients cohort, who cannot undergo open repair.
