Aim: Authors analyze criteria for the use of PTFE (Propaten®) as first choice conduit by retrospective investigation of patency rate in patients with low thrombogenic risk. Saphenous vein was intended to be spared for distal revascularization or subsequent redo bypass.
Methods: Between January 2003 and December 2012, 212 patients were selected at our department to undergo subpopliteal revascularization by PTFE prosthesis. In the same period of time 238 patients who did not meet our criteria received a vein bypass. In few cases a composite graft (PTFE + vein) was constructed if poor saphenous vein was observed. All 450 patients data were listed in the Registro Italiano Propaten.
Results: PTFE (Propaten®) graft employed for subpopliteal revascularization significantly reduces time related surgical risk. Long-term outcome proved excellent graft performances (primary patency 40% and secondary 50% at nine years) with a 90% limb salvage rate at nine years.
Conclusion: Authors deem medical (associated diseases, clinical status) and anatomical criteria as satisfactory in patient selection, they also hope for future improved standardized methods.
