Lower extremity peripheral arterial disease. Are cutting balloons the best treatment for peripheral arterial disease?

Cutting balloons have been available for a decade but have not achieved a clear place in endovascular treatment. This review considers the data available for its use in the peripheral arterial circulation of the lower limb. Data is conflicting. The balance of evidence is that cutting balloon angioplasty confers no advantage over other endovascular techniques for atheromatous stenosis or occlusion. However in situations where stenosis is due to neo-intimal hyperplasia, such as in-stent restenosis and graft stenosis, there is a reasonable evidence base for their successful use as a first-line endovascular therapy.
