We ­present a ­rare ­case of undi­ag­nosed phae­och­rom­o­cy­to­ma fol­low­ing infra­in­gui­nal ­bypass sur­gery. The ­patient, a 59-­year-old ­lady, had a one ­year his­to­ry of hyper­ten­sion fol­low­ing a ­first femo­ro-tibi­al ­bypass and pre­sent­ed as a car­di­o­res­pir­a­to­ry emer­gen­cy in the admis­sion ­room fol­low­ing her con­tra­lat­er­al femo­ro-tibi­al ­bypass. The ­patient recov­ered ­after ­some ­days in inten­sive ­care ­despite a ­delayed diag­no­sis.
