Myoepithelioma is a rare benign neoplasm which can occur in either the major or minor salivary glands. Extrasalivary myoepithelioma are uncommon, with ten cases reported in the sinonasal cavity. We present a complete review of the literature regarding sinonasal localization and we report a new case of myoepitelioma arising from the nasal septum. A 67-year-old man presented with right sided nasal obstruction and recurrent epistaxis, due to a nasal myoepithelioma, which is endoscopically resected with diode laser. Histopatological examination showed a predominant solid pattern of spindle shaped cells; however a minor component of reticular pattern and islands of plasmocitoid cells were present. Immunohistochemistry was important for the diagnosis. No evidence of tumor recurrence was seen in the 3 months following surgery. Nasal myoepithelioma represents a rare tumor. Complete resection is important to avoid recurrence. Endoscopic excision by means of diode laser can represent a safe procedure with good results.
