Pigmented vil­lo­nod­u­lar syn­ovitis (­PVNS) is a dis­ease of the ­joints ­which uncom­mon­ly ­involves the ­spine. We ­present a 70-­year-old wom­an ­with radic­u­lar symp­toms who was ­found to ­have a ­mass aris­ing ­from a lum­bar zyga­poph­y­seal ­joint ­with exten­sion ­into the spi­nal ­canal. Following ­gross-­total exci­sion of the ­mass, his­tol­o­gy ­revealed ­PVNS. One ­month ­after sur­gery, the ­patient had no symp­toms and ­there was no evi­dence of resid­u­al or recur­rent dis­ease.
