Avascular necrosis is a common disease which typically affects younger patients and induces the deterioration of weight-bearing joints. An early diagnosis, especially in the pre-collapse of articular surface, is crucial for the efficient treatment, because most of the untreated cases progress to total hip arthritis. The goals of treatment are to alleviate pain, decrease necrosis or stop progression. There has recently been an increasing concern about joint-preserving methods used in the pre-collapse level with the aim of preventing femoral head collapse. Various joint preserving methods have been described in the literature, however controversy still exist regarding which should be regarded as the most efficient method. Moreover, agreement also lacks regarding the ideal management algorithm, as surgeons follow different treatment strategies according to their own clinical experience and knowledge. This review aims to present hip-preserving methods in the treatment of avascular necrosis of femur head.
