Hemiarthroplasty versus internal fixation for intertrochanteric fractures in elderly patients

AIM: The aim of this paper was to explore the clinical efficacy of hemiarthroplasty and internal fixation in treatment of elderly patients with unstable intertroehanteric fracture.
METHODS: A total of 127 elderly patients with unstable intertroehanteric fracture were divided into hemiarthroplasty group (N.=69) and internal fixation (N.=58) by completely randomized method. The operative time, blood loss, blood transfusion units and the rate of postoperative complications of the two groups were compared, and the Harris Score and two years after surgery were evaluated.
RESULTS: The operative time in hemiarthroplasty group was shorter than that in other group, but blood loss and blood transfusion units in hemiarthroplasty group were also greater than those in other group (P<0.01); the differences in rate of postoperative complications, Harris Score of joint functions were not statistically significant between the two groups (P>0.05).
CONCLUSION: The hemiarthroplasty and internal fixation group are effective in treatment of elderly patients with unstable intertrochanteric fracture. Hemiarthroplasty is a option for unstable intertrochanteric fractures in elderly patients with severe osteoporosis.
