Assessment and comparison between 2 treatment methods: fixed locking angle plate and cannulated screws, in type 3 Pauwel femoral neck fracture

AIM: Femoral neck fracture is a debilitating injury which is highly prevalent in elderly patients. In this study we evaluated and compared 2 treatment methods: fixed locking angle plate and cannulated screws in type 3 Pauwels femoral neck fracture.
METHODS: This is a prospective comparative study carried out in 2010. During the follow-up visits in the third and ninth months, VAS and Harris Hip questionnaires were filled in and the rate of fracture union in two groups was recorded. Moreover, at the final visit the patients’ quality of life was evaluated according to the SF-36 questionnaire. Data analysis was conducted with SPSS 18.0 and P<0.05 was considered significant.
RESULTS: Changes in Harris Hip Score from the third month to the ninth month were significant in both groups, and changes were significantly different in the first group in comparison to the second one (P=0.042). In 4 patients (13.33%) treated with cannulated screws non-union occurred, and in 1 patient from locking plate group non-union was recorded. Changes in Visual Analog Score from the third month to the ninth month were significant in both groups, and changes were significantly different in the first group in comparison to the second one (P=0.005).
CONCLUSION: Regarding better hip function and better mental and emotional effects, the fixed locking angle method is a better approach to treat Pauwels type 3 fractures than the treatment by cannulated screws.
