Recognizing the importance of small recurrent radial artery with radial artery loop and its impact on the outcome of transradial procedure

Trans radial approach is becoming an increasingly common technique for coronary angiography due to a reduced risk of vascular complications. Anatomical variations of the radial artery can sometimes result in procedure failure, specifically with an unrecognized radial artery loop leading to wire induced perforation. Sixty-three year old male who underwent coronary angiography via the radial artery felt extreme pain when the catheter inadvertently entered the small recurrent radial artery. Catheter was withdrawn and hydrophilic guide wire was advanced with care under fluoroscopy avoiding recurrent radial artery. Catheter was then advanced over the guidewire and procedure was completed.
