Giant aneurysmal change of a pulmonary sequestration artery in 73-year-old man

Intralobar sequestration (ILS) is the dominant type of pulmonary sequestration. Although diagnostic delay is common in adults, ILS is rare in elderly patients. A long-standing, aberrant systemic artery, supplying the sequestrated lung, may dilate and develop an aneurysm with a high risk of rupture. An endovascular procedure is recently developed as feasible alternative to surgery in selected cases. We present a 73-year-old patient with a giant aneurysmal systemic artery originating from the thoracic aorta who was successfully managed with preoperative thoracic endovascular aortic repair, preventing possible perioperative rupture of the aneurysm and bleeding from inadvertent arterial injury during resection.
