One-port one-window (OPOW) technique in thoracoscopic surgery

BACKGROUND: In video-assisted thoracoscopic surgery (VATS) using two incisions, there could be some limitations of vision field and ergonomic discomfort in manipulation. With using one-port one-window (OPOW) technique with insertion of camera through the working window (WW), we could overcome these difficulties.
METHODS: This retrospective study involved seven patients who underwent OPOW at our institute from May 2012 to May 2014. The medical records were retrospectively reviewed.
RESULTS: Of seven patients who underwent OPOW, six cases with lobectomy, one case with segmentectomy and two cases with wedge resection were performed. Comparing lobectomies using OPOW technique with lobectomies using conventional VATS in similar conditions, there were no significant differences in duration of chest tube placement, hospital stays and operative time. Rather, the mean operative time was significantly shorter in OPOW group (P=0.006) than conventional group. There was no occurrence of major perioperative morbidity and mortality in OPOW group.
CONCLUSIONS: Although a few cases have been performed using OPOW in our hospital, there were no significant differences comparing with conventional VATS lobectomy. OPOW technique with insertion of camera through the WW could be one of safe options in the surgical approach to pulmonary diseases.
