We report a case of a rare left ventricle posterior side aneurysm in adult. A 69 year old male patient administered to our department with chest pain. Physical examination revealed a mild diastolic murmur. The electrocardiogram showed a normal sinus rhythm by 80 min. Arterial blood pressure was 140/90 mmHg. Lungs and abdomen were clear to auscultation. By the preoperative evaluation, transthoracic echocardiography showed a mild mitral valvular regurgitation. Coronary angiography presented a large true aneurysm of left ventricle posterior face combined with severe three vessel coronary stenosis. Patint underwent to three vessel coronary surgery with lineer aneurysmectomy. Postoperative follow-up was uneventful. Patient was discharged at the postoperative eleventh day.
