Inguinal hernia repair: accidental finding of liposarcoma

We describe the case report of a patient who came to our attention presenting a painless swelling localized in the right groin. The patient has been sent to our surgical departement with the diagnosis of inguinal hernia in order to perform a inguinal hernia repair with polipropylene prothesis. At the beginning of the surgery, a cerebroid, hard-wood consistent, hypervascularized mass has been found in the usual localization of the prehernia lipoma. These operative finding, radically removed and histologically analyzed, has been demonstrated a poorly differentiated soft tissue sarcoma with both adipogenic and rabdomuscolar components (immunoistochemistry: myogenin +, desmin +, MDM2 +, CD34 +, atina1A4 -, S-100 protein +, CD57-). Because of the described clinical and anatomopathological diagnosis, the patient has been then contacted in order to start as soon as possible the diagnostic-stadiative process and for the setting of a proper neoplasm therapy. Thus, a total-body CT scan with contrast medium has been performed showing no other tumoral repetitions and an oncological follow-up has been started.
