Parotid gland surgical treatment: outcomes and complications

Aim and methods. This paper presents a retrospective study carried out on a sample of 36 patients affected by different parotid gland pathologic entities surgically treated at the Department of Maxillofacial Surgery of Messina University, G. Martino Policlinic, between 2001-2007. The median age of the patients was 55.3 years, and the group consisted of 27 males and 9 females. Accurate anamnesis and clinical examination was performed in all patients. Preoperatory diagnosis was assisted by ultrasound, ultrasound guided fine needle aspiration, CT with and without contrast and MRI.
Results. Twenty-seven out of 36 lesions were categorized as benign lesions: 23 cases had pleomorphic adenoma, three cases had Warthin’s tumor and one had oncocytome. Other tumors diagnosed were: 1 mucoepidermoid carcinoma, 1 squamocellular carcinoma, 1 myoepithelioma, 2 recurrent pleomorphic adenomas, 1 nodular lesion with epitheliomorphic epidermoidal elements, 1 Kaposi’s sarcoma, 1 hemangioendothelioma, 1 hyperplastic reactive intraparenquimal lymphonode. A systematic review of the literature about surgical techniques, outcomes and complications of parotid surgical treatments is provided.
Conclusion. A multidisciplinary approach is desirable, for their treatment in order to analyze schematically the outcomes, early recognize possible complications and establish a correct treatment that reduces the time of healing and patient discomfort.
