Pop­li­teal ­artery entrap­ment syn­drome (­PAES) is an ­uncommon ­cause of arte­rial insuf­fi­ciency, espe­cially in ­younger, ath­letic ­patients. Sur­gical treat­ment ­should be per­formed in all ­cases of ­PAES ­whether the ­artery is ­occluded or not. ­Although tran­sec­tion of the com­pressing ­muscle or fas­cial ­band is ­required to ­obtain ­good ­results, con­tro­versy ­exists ­about recon­struc­tion of the tran­sected ­muscle or fas­cial ­band. We ­present a ­case of a ­young ath­lete ­with ­PAES for ­whom ­rapid ­recovery of the ath­letic ­activity was pro­vided by recon­struc­tion of the ­divided ­medial ­head of the gas­troc­ne­mius ­muscle.
