Is there a place for surgery in acute type B aortic dissection?

Surgery has been indicated for complicated acute type B aortic dissection. The new endovascular approach by stent-grafting has demonstrated a reduction of mortality and morbidity. However, no robust evidence about its superiority is available. General experience considers endovascular treatment beneficial for such pathology. The better knowledge about the natural history and predictor factors in addition to new dedicated endografts can consolidate and expand the endovascular treatment as a good therapeutic strategy for complicated acute type B dissection and even for uncomplicated cases.
