A case of lower limb acute ischemia after application of the Angio-Seal vascular hemostasis system

We ­report a ­case of low­er ­limb ­acute ische­mia fol­low­ing appli­ca­tion of the Angio-Seal per­cut­ane­ous hemo­stat­ic ­device. The ­patient, who had ­been stud­ied ­with dig­i­tal sub­trac­tion angio­gra­phy ­prior to carot­id endar­te­rec­to­my, was ­again ­referred to our atten­tion 5 ­days ­after dis­charge ­from hos­pi­tal ­with ­signs of low­er ­limb ­acute ische­mia. Anamnestic and objec­tive sourc­es of embol­isa­tion ­were exclud­ed. The ­only ­risk fac­tor con­sist­ed of the pre­vi­ous appli­ca­tion of the Angio-Seal ­device on the femo­ral ­artery. Emergency angio­gra­phy and intra­op­er­a­tive find­ings dur­ing throm­boem­bo­lec­to­my ­showed dis­tal embol­isa­tion ­from the ­anchor of the hemo­stat­ic ­device. In the ­light of ­this ­case and a ­review of the lit­er­a­ture, we con­firm the undoubt­ed util­ity of the Angio-Seal ­device, but we ­believe ­that ­there is the pos­sibil­ity of ­device-relat­ed com­pli­ca­tions.
