Extremity vascular traumas. A report of 22 cases treated

Background. An increase in the number of vascular traumas of the extremities has been observed over the past decades causing many problems for a multidisciplinary therapeutic approach and surgical technique.
Methods. The ­authors ­report a ­series of 22 vas­cu­lar trau­mas treat­ed ­over a 42-­month peri­od. No fatal­ity was report­ed. Sixteen ­patients (72.7%) under­went sim­ple eco ­colour-Doppler direct­ly in the oper­at­ing thea­tre, where­as angio­gra­phy was ­only ­used in 6 cas­es of low­er ­limb ­lesions asso­ciat­ed ­with mul­ti­ple trau­ma (27.2%). Limb sal­vage was ­achieved in 21 cas­es (95.4%); demol­i­tive sur­gery was ­only ­required in 1 ­case (4.5%). Severe neu­ro­log­i­cal seque­lae ­were ­observed in 2 cas­es (9%) and ­motor def­i­cit ­caused by ten­don ­lesions in 1 ­case (4.5%).
Results. The ­good ­results ­obtained are the ­result of the ­short ischem­ic inter­val ­between the ­acute ­event and treat­ment (­mean ­time ­lapse 3 ­hours and 40 min­).
Conclusions. The ­authors dis­cuss the ­need for a mul­ti­dis­ci­pli­nary ­approach ­that is as rap­id as pos­sible to guar­an­tee the ­best ­long-­term ­results.
