Analysis of the static and dynamic postural control in multiple sclerosis

Aim. The 5% of subjects with multiple sclerosis in remission chronic form suffers from equilibrium disorders consisting in static and dynamic postural control impairments. The purpose of the research is to investigate these impairments in order to individuate a postural pattern leading to diagnostic suspect before imaging investigations.
Methods. Eighty patients (45 male, 35 female; average age ± SD: 28±4,5; range 22-44 years old) with multiple sclerosis and postural impairments were recruited. Each patient was subjected to anamnesis, otologic and otoneurologic clinical examinations and to a postural evaluation in static and dynamic conditions through Equi test.
Results. The sensorial analysis has disclosed a prevalent alteration of the somatosensorial index, abnormal in 82.5% (66/80) of the cases, compared to the other parameters and a stabilizing effect of visuo-proprioceptive deprivation whereas the strategy analysis has shown the use of hip strategy in all the subjects with grave unbalance. In the motor control test a meaningful deficit of the flexor postural program was observed.
Conclusion. The findings of the present study suggest that the diagnosis of multiple sclerosis in initial phase can be supported by posturological investigations and that the most important postural signs in these cases are in a number of three: the proprioceptive index impairment, the stabilizing effect of visuo-proprioceptive deprivation and the flexor postural program deficit.
